Incident & lost property report

One form for two things: something that happened in a SYD CABS taxi, or something you left in one. Choose which at the top of the form — the questions change to suit.

Reporting an incident

An accident, an assault or a threat, unsafe driving, or anything else that affected someone's safety or wellbeing during a trip.

We ask because SYD CABS has to investigate it, and because some incidents are notifiable occurrences that we are required to report to the Point to Point Transport Commissioner. We cannot report what we have not been told. The record is kept for seven years.

Reporting lost property

Something left behind in a taxi — a phone, a bag, keys, a wallet.

The detail is the whole job here. Matching an item to the right trip depends on the taxi number, the date and roughly when you travelled, so give us whatever you have. A clear description is what tells your item apart from somebody else's.

On the roof sign, the doors, and your receipt. Leave blank if you did not catch it.

Accepted: JPG, PNG, PDF. Photographs of damage, an injury, a receipt or the vehicle all help.

If someone is in danger

This form is not monitored in real time. If there is an emergency, or a crime is in progress, call 000 first and report it here afterwards.

How your report is handled

You get a reference number straight away and an acknowledgement by email. Where a report involves a driver or a vehicle, we ask them for their account before any finding is made. Camera footage is secured as soon as a report reaches us.

Overcharged instead?

If this is about the fare — an overcharge, a refused fare, or a refund — use the fare complaint form. It goes to the same team but captures the trip details we need to check the meter.
