Driver version form

Your written answer to a complaint. You are told what has been alleged and given the chance to respond before any finding is made — this is how you do it.

On the letter or email asking for your version. It starts with SC- for a fare complaint or SI- for an incident. If you cannot find it, call 1300 926 117 rather than guessing — a wrong reference means your statement is not attached to the right matter.

Take your time. This is read before any finding is made, and it is the record of your account.

Meter photographs, dockets, screenshots, dashcam stills. Accepted: JPG, PNG, PDF.

This is my own account of what happened and it is true to the best of my knowledge. I understand it is kept on file for seven years and may be given to the Point to Point Transport Commissioner or to Police if the law requires it.
Thanks — your statement has been logged.

Why we ask

Before any finding is made about you, you are told what has been alleged and given the chance to answer it. That is your right under the Complaint Handling Policy, and this form is how you use it.

Please respond within 3 business days

If we do not hear from you in that time we may decide the matter on the evidence we have. It is far better to answer late than not at all — send it anyway.

You can ask for a review

If a complaint is upheld and you disagree with the outcome, you can ask for it to be reviewed by a manager who was not involved in the decision, within 14 days.
